This form is required creating new student accounts for the school’s Cafeteria POS system.  Please note that some schools automatically provide new account data to Cezars Kitchen.

Cezars Kitchen charges a one-time new account registration and setup fee of 1000yen. This is automatically applied to the new account at the time of first prepayment.
